A multicenter, randomized, clinical trial, sponsored by the Indian Stroke Clinical Trial Network (INSTRuCT), involved 31 centers. Eligiblity determination was performed for 5640 patients between April 28, 2018 and November 30, 2021. Randomly allocated to either the intervention group (n=2148) or the control group (n=2150), a total of 4298 patients participated in the study. Diarrheal illness, a major global contributor to childhood morbidity and mortality, has Shigella as a key causative agent, for which a potential vaccine is currently under consideration. A key goal of this research was to create a model depicting the changing patterns of paediatric Shigella infections over time and space, and predict their prevalence in low- and middle-income nations.
Studies on children aged 59 months or less, located in low- and middle-income countries, contributed data for individual participants demonstrating Shigella positivity in stool samples. Factors at both the household and individual participant levels, as determined by the investigators, were included as covariates, along with environmental and hydrometeorological variables obtained from numerous georeferenced data sources for each child's location. The fitted multivariate models provided prevalence predictions, further categorized by syndrome and age stratum.

Our prospective, observational study (IDAMS) encompassed patients aged five years and above who presented with undifferentiated fevers at 26 outpatient clinics distributed across eight nations, specifically Bangladesh, Brazil, Cambodia, El Salvador, Indonesia, Malaysia, Venezuela, and Vietnam. Multivariable logistic regression was utilized to explore the connection between clinical symptoms and laboratory findings in dengue versus other febrile illnesses, occurring between two and five days after the onset of fever (i.e., illness days). We generated a selection of candidate regression models, including those derived from clinical and laboratory measures, aiming for a balance between comprehensiveness and parsimony. We evaluated the performance of these models using standard metrics for diagnostic accuracy.
Between October 18, 2011 and August 4, 2016, the study population comprised 7428 patients.
